Patient Forms

You can save time and expedite your first visit by downloading and completing the forms in our “New Patient Package” at home. Simply hand the forms to our receptionist at check-in. Also, remember to bring your insurance card and an ID with you to your first appointment.

What to Expect at Your First Visit

At your first visit, you will be welcomed by an experienced and caring staff ready to address your dental concerns. Once we gather the relevant background information, obtain diagnostic images and perform a clinical evaluation then we will make a plan for you, even if it is just to schedule your next cleaning. We want you to have the best possible experience, and look forward to meeting you.
